COMPASSIONATE SENIOR DAYTIME CARE

OC Care Connections Outreach is specifically designed for seniors who are frail, require extra care and supervision, or diagnosed with mild to moderate memory impairment.

Respite care is available weekdays, 9:30AM - 3:30PM and, on the first Saturday of the month, 9AM - Noon, allowing you the opportunity to attend our support group.

Eligibility will be determined prior to admission.

To be eligible for the program, participants must be:
- Ambulatory
- Continent
- 55 years or older
- Under the care of a physician
- Tested for TB within the past 12 months
- Diagnosed with mild to moderate memory impairment and other related disorders, or frail requiring supervision or care.

How We Can Help You
Family members need respite from the physical and emotional demands of 24-hour care. At the same time, diverse activities, attention and interaction with others are healthy for the elderly. OC Care Connections Outreach provides affordable care that meets the functional, creative, emotional, and spiritual needs of the elderly. This program is open to anyone regardless of religious affiliation.
